Now, you've turned in your child's physical, shot record and blue health form and the Certified School Nurse (CSN) wants more information. Why?

Your CSN may contact you for clarification of your child's health issues to determine the possible impact on their ability to learn. This information may be shared with your child's classroom teacher and art, music, PE teachers. In some instances the information will be shared with lunchtime personnel, for example a bee sting allergy or food allergy.

During the year, please inform the CSN of any changes in your student's health status such as a new diagnosis or new medication. Working together, we can help your student have a healthy, successful school year.
