Steps for School/Site/Department Based Professional Development Application
-Download the application (Word document file, bottom of page).
-Follow the Guidelines for Application (pdf file, bottom of page).
-Include all the required information.
-The Word document is a fillable document. You may type into the document AFTER you download it. Rename and save your completed document.
-Print your completed application.
-Gather the required signatures.
-Scan and email the signed application to the Professional Development Committee email, firstname.lastname@example.org. Applications MUST be submitted electronically on the most recent application form (CU-PD-F004).
-Applications are reviewed during Professional Development Committee meetings using the Guidelines. If your application is not approved, feedback will be provided based on the Guidelines (CU-PD-F005). The committee encourages revisions and re-submissions.
