School District U-46 is committed to ensuring the safety of all employees, students and visitors. Any employee injured while at work must report the incident immediately to a supervisor or Human Resources, and an Employee Accident/Injury Report must be forwarded to Human Resources via fax (847-888-6990) or email (Benefits@u-46.org) within 24 hours of the incident.
Sedgwick Claims Management Services (SCMS) investigates and processes all work-related injuries. All treatment after initial emergency care must be reviewed and authorized by SCMS.
Any time off due to a work-related injury or illness may be eligible for leave under the FMLA (Family Medical Leave Act), and therefore the district is required to notify and offer this benefit. If the leave qualifies, the FMLA would run concurrently with the workers compensation leave. If eligible for FMLA leave, up to 12 weeks of leave are available and additional information related to the Act will be provided.
