A reminder for all 8th grade parents. Your child will require a 9th grade physical complete with immunizations as part of the registration process for the 2019-2020 school year.  All physicals MUST be on the State of Illinois Certificate of Child Health Exam form dated 11-2015. NO other forms will be accepted. New for 19-20 school year, all freshman students will be required to turn in proof of a dental exam by May 15th , 2020.  Students will not be able to print schedules or attend school on the first day unless a current physical and immunizations have been turned into the high school they will be attending (Eastview cannot accept the physcials.)  Parents are encouraged to schedule their child’s physical as soon as possible. Thank you.
