SHIELD Testing Update 04/2022
As of April 4th, 2022 our drive-through testing location will no longer be available on Tuesday. The drive-through (located at 955 E. Chicago St. in Elgin) is open on Monday's and Friday's (when the District is open) from 7:30am-10:00am and again from 1:00pm to 6:00pm. This site is available for students and staff members who are currently enrolled in the SHIELD Screening Program.
SHIELD Testing Update 02/2022
As of February, 2022 SHIELD testing will be open to all students and staff regardless of vaccination status. If you are interested in signing up for the SHIELD screening program, please complete this online consent form.

Physical and immunization requirements for the 2022-23 School Year
Per School Board Policy 7.100, all students will be compliant with State law required health examinations and immunizations on the first day of school. Students who are non-compliant in their physical and immunization requirements will not be allowed to attend school until state compliance is met. Required grades include Preschool/Early Childhood, Kindergarten, 6th, 9th, and 12th grade.
- Vision exams for Kindergarten are required by October 15th, 2022.
- Dental exams for Kindergarten, 2nd, 6th and 9th grade are due by May 15th, 2023.

If you have completed physical and immunization forms, they can be submitted as a PDF scanned document via email to email@example.com. Parents also have the option to upload their required health documents as a PDF through Infinite Campus during the registration process.
Forms can also be submitted directly to your child’s school or via email to the nurse within the school. Please contact the school health office for the email address of your school nurse.
Any updates from the Illinois State Board of Education will be communicated to all parents.
