We are new to District U- 46. What documents are needed?

If you are transferring to U-46 from a different district (regardless of where the district is located), you must meet the minimum health requirements to start school. Students entering kindergarten, 6th grade and 9th grade need a physical completed within the past year. Students entering in a different grade must meet the minimum requirements for the last rquired grade (eg. incoming 2nd graders must meet the Kindergarten health requirements).

Students enrolled in school after the first day of school have 30 days to meet the minimum health requirements of the applicable requirement year.
